About The Job
As a pivotal member of our multidisciplinary team, the Clinical Pharmacist delivers exceptional clinical pharmacy services in both acute care and residential care settings. Your role is an Antimicrobial Stewardship role which involves personalized treatment assessments, serving as the primary drug information resource, and offering expert advice to healthcare professionals. You'll meticulously monitor drug prescribing, distribution, and therapy to ensure their safe, effective, and economical use.
Embracing a patient-centered approach, you'll oversee ongoing drug therapy, assessing factors like effectiveness, compliance, adverse reactions, and interactions. Your responsibilities include maintaining accurate workload records, communicating vital findings and recommendations to the healthcare team, and ensuring meticulous documentation in patient health records or computerized systems. Additionally, you'll interpret medication orders, input them into the database, and dispense prescriptions in strict adherence to legal, professional, and departmental standards. This is a Permanent Full Time (1.00) opportunity, working rotating day and evening shifts.
