Qualifications
- Registered and in good standing with the College of Dietitians of Ontario
- Must provide proof of:
- Liability insurance
- TB testing
- Vulnerable sector check
- Ability to work both independently and within a multidisciplinary team
- Strong written and verbal communication skills
- Excellent organizational and time management abilities
- Demonstrated problem-solving skills
- Proficiency with computers, including experience using electronic medical records (EMR)
- Travel may be required for certain positions
- Identify and monitor the level of nutrition and hydration risk for all Residents
- Complete scheduled RAI assessments: on admission, quarterly and significant status changes
- Prioritize and respond to dietary referrals in a timely manner
- Develop, evaluate, and modify individualized nutrition care plans
- Liaise with Residents, families/SDMs, and the team to implement effective dietary interventions
- Conduct swallowing assessments as required
- Provide nutrition counselling and education to Residents and their families
- Collaborate with the Nutrition Manager to enhance the dining experience
- Participate in Care Conferences and committee meetings as per organizational needs
- Review and approve seasonal menus, and create individualized menus where applicable
- Provide training and support to dietary and nursing team
- Contribute to continuous quality improvement (CQI) initiatives
- Ensure all work complies with Ontario Regulation 246/22, Section 80, under the Fixing Long-Term Care Act, 2021
- Perform additional duties as required
