Detailed Overview
Reporting to the Site Coordinator or designate, the Licensed Practical Nurse (LPN), practices in accordance with the standards of professional practice as well as utilizing a client and family centered care model and the vision and values of the department within Fraser Health (FH).
The LPN practices within the context of a client and family centered care model and in accordance with the British Columbia College of Nurses and Midwives (BCCNM) standards of practice for Licensed Practical Nurses. Working independently and collaboratively as a member of an interprofessional care team, performs full scope of practical nursing functions for clients with predictable outcomes within a designated area of specialty. This includes activities such as assessing, planning, implementing, evaluating and documenting client care, and providing support to clients and their families. Working collaboratively as a member of an interprofessional care team and under general supervision, supports the Registered Nurse in caring for clients with unpredictable outcomes or where the acuity or complexity of care requires an advanced level of knowledge and skill beyond the scope of practice for Licensed Practical Nurses. Maintains and advances clinical competence pursuant to the standards of practice of BCCNM for Licensed Practical Nurses.

Graduation from a recognized program for Practical Nurses or an equivalent combination of education, training and experience. Current practicing registration as a Licensed Practical Nurse with the British Columbia College of Nurses and Midwives (BCCNM).

Responsibilities
- Establishes therapeutic relationships with clients and families. Assesses and monitors the physiological, psychological, sociocultural and spiritual needs of stable clients, and develops and maintains an individualized plan of care based on the assessed needs of stable clients and their families.
- Provides and delivers nursing care to stable clients consistent with the written care plan, physician orders and in accordance with the standards of BCCNM for LPNs, and established procedures by methods such as administering selected medications, applying and changing simple sterile and/or non- sterile dressings, taking vital signs, suctioning of tracheostomy, maintaining tube feeds, monitoring and discontinuing of IVs, inserting urinary catheters, and providing personal care to clients by assisting with activities of daily living.
- Uses up to date nursing practice knowledge to select and individualize nursing interventions for stable clients within the parameters of the care plan and consistent with client/family priorities. Monitors status of clients in relation to anticipated outcomes. Evaluates the effectiveness of nursing interventions by comparing actual outcomes to anticipated outcomes. Reports unanticipated changes and concerns to the Registered Nurse and/or Charge Person and modifies the care plan as appropriate in collaboration with the Registered Nurse according to evaluation findings.
- Works in partnership with the Registered Nurse and other care givers as an interprofessional member of the care team to meet the care needs of stable clients and their families. Supports the Registered Nurse in caring for clients with unpredictable outcomes or where the acuity or complexity of care requires an advanced level of knowledge and skill beyond the scope of practice for Licensed Practical Nurses by methods such as performing practical nursing activities, reporting changes and concerns, and collecting of equipment and supplies as required.
- Involves the client/family in planning and implementing care by performing duties such as explaining nursing interventions, utilizing therapeutic communication, providing incidental health teaching during routine care and responding to client requests by establishing timeframes, ensuring other priorities are met and acting as an advocate to promote the client/family right to autonomy, respect, dignity and access to information.
- Identifies client/family priorities/plans and contributes to the revisions of the nursing care plan as required for stable clients; includes client/family in evaluating progress towards their goals.
- Documents client/family perspectives, nursing observations, and activities that support client/family priorities. Maintains complete and accurate computerized and paper records through methods such as inputting data and documentation in accordance with established procedures and policies.
- Demonstrates and/or provides guidance on health and personal care related tasks to designated care givers and guides the clinical experience of assigned students and new staff; provides input to the Manager as requested on individual's progress in achieving established standards of client care.
- Uses appropriate supplies and equipment for client care in accordance with departmental standards regarding cost effectiveness, availability, and safe care requirements. Problem-solves equipment malfunctions with technical staff. Handles, stores, and disposes of workplace hazardous materials and sharps in accordance with established procedures.
- Participates in unit/team activities required to enhance comfortable and safe delivery of care, through methods such as promoting quality management principles for unit/team operations, identifying processes/forms which require improvement, re-stocking supplies and attending to minor spills. Identifies and reports unsafe practice/professional conduct in accordance with professional standards and hospital policies.
- Participates in analyzing, developing, implementing, and evaluating nursing practice and policy on designated unit and as part of the interprofessional team. Plans, implements and evaluates own work pattern by organizing, prioritizing, and setting time frames, and adjusting these within the team to reflect client and unit priorities. Participates in unit discussions and in-services through methods such as attending meetings to provide information on the planning of care and development of the nursing care plan for clients.
- Participates in approved nursing and interprofessioinal research and special projects on assigned units in conjunction with team members, through methods such as collecting data, integrating and interpreting data, summarizing information, and/or providing verbal updates for review and further analysis by the project coordinator.
- Sets measurable goals and objectives within a team or individual environment, incorporating the organizations vision and values, goals and corporate plan through methods such as maintaining licensure, updating clinical knowledge and competencies and developing a plan for professional development in collaboration with designated personnel/team pursuant to the standards of practice of BCCNM for Licensed Practical Nurses.
- Responds to incidents such as missing clients, injuries to staff or visitors, Code Blue/White emergencies, hazardous spills, fire and aggressive/disruptive behaviour to ensure medical and other support is accessed in a timely manner; performs cardiopulmonary resuscitation, informs designated staff of occurrences, and completes forms in accordance with established procedures and policies.
- Carries out responsibilities in accordance with health and safety requirements. Immediately reports unsafe situations by notifying supervisor or other appropriate personnel.
- Performs other related duties as assigned.
Education, Training and Experience
