PCH currently operates two new state-of the-art LTC homes and community hub in West Mississauga. The new homes have a combined 632-beds and be part of PCH’s larger strategy that introduce innovative and inclusive programs and services and new models of care delivery. Position Summary:
PCH is seeking a Clinical Lead to support the Behaviorol Support Unit (BSU) at Partners Community Health, a transitional home area that provides specialized care for people with responsive behaviours associated with a dementia and older Adults with Mental Health Disorders. As a Clinical Lead, you will be providing exceptional healthcare services within the Long-term care home. The Clinical Lead position will be reporting to the Associate Director of Care. The ideal candidate would be a Registered Practical Nurse with strong leadership skills, clinical expertise, and a commitment to promoting equity and inclusion in healthcare.

Position Summary:
PCH is seeking a Clinical Lead to support the Behaviorol Support Unit (BSU) at Partners Community Health, a transitional home area that provides specialized care for people with responsive behaviours associated with a dementia and older Adults with Mental Health Disorders. As a Clinical Lead, you will be providing exceptional healthcare services within the Long-term care home. The Clinical Lead position will be reporting to the Associate Director of Care. The ideal candidate would be a Registered Practical Nurse with strong leadership skills, clinical expertise, and a commitment to promoting equity and inclusion in healthcare.
Key Responsibilities:
- Provide sound clinical knowledge and skills, act as clinical expert and resource, and provide comprehensive, quality client/family-centred health care services
- Collaborate with Psychogeriatric Resource consultants and mobile behaviour support resources in the development/refresh of the LTCH’s Behaviour Support Resource Team (BSRT)
- Provide leadership and support to the LTC home’s BSRT
- Coach LTC home staff in the use of frameworks such as PIECES and UFIRST to assess responsive behaviours, identify triggers and develop behaviour care plans and approaches to providing care to residents requiring behavioural support as well as their families
- Maintain a short-term active clinical caseload of older adults presenting with or at risk for responsive behaviours that may be associated with dementia, complex mental health, substance use and/or other neurological conditions
- Provide comprehensive bio-psychosocial geriatric assessments, treatment, follow-up and discharge planning in collaboration with the BSRT
- Provide coaching on the implementation of behaviour care plans and guide appropriate care plan transitions
- Assist in the transition of long-term care residents with responsive behaviours (new or existing) between community, hospital and neighbouring long-term care homes (LTCHs)
- Collaborate and communicate with BSRT members, Geriatric Mental Health Outreach Team, Psychogeriatric Resource Consultant, Long Term Care Behaviour Support Outreach Team, hospitals, CCAC, primary care and other health care professionals
- Convene and chair regular Behaviour Rounds that enable front line staff to discuss care plans with internal and external resources
- Identify the need for additional consultation and/or referrals to external services or specialties and coordinate referrals
- Assist in the implementation and sustainability of care plans and recommendations of external resources by providing coaching to LTCH staff
- Document all findings and interventions in a clear and concise manner in the client’s clinical record
- Document statistics and indicators as part of BSSP and submit regular reports
- Advocate for and, protect the individual rights of residents/clients
- Raise client-specific challenges/barriers to supervisor, as a means of identifying systemic issues where broader consultations may be required
- Maintain a current knowledge of Alzheimer’s disease and related dementias, mental health and addictions and neurological conditions
- Proactively identify training and education needs for the home and coordination with the PRCs to deliver capacity building supports
- Contribute to BSRT Lead community of practice
- Provide expert clinical assistance to neighbouring LTCH’s
- Performing other duties as assigned
- Registered Practical Nurse with current registration with the College of Nurses of Ontario
- Minimum one (1) year experience with clients in long-term care or residents in a long-term care home setting
- Current CPR/First Aid certification
- Training in Gentle Persuasive Approaches (GPA)
- Demonstrated knowledge and skill working with older adults presenting with or at risk for responsive behaviours that may be associated with dementia, complex mental health, substance use and/or other neurological conditions
- Demonstrated ability to train, coach, and mentor others
- Demonstrated ability to take professional accountability for leading, organizing, providing and delegating care activities for residents and families
